Keeping track of your medications can be a hassle!

Especially when you have to carry around paper logs, or worse, use a complicated app.

The Simplest Medication Log is an easy way to keep track of your medications, side effects, and more all in one place, on the device you already have on you, in an app you probably already use, and only takes a moment to add a new entry allowing you to move on with your day!

It is a duplicatable Notion template for keeping a log of medications and dosages you're taking, with fields for how they make you feel and any side effects or other notes you want to keep track of. There are very few fields to make logging fast and hassle-free! There are separate Timeline, Log, and Table views that you can print or directly share with your doctor. No paper, just the simplest medication log you can find!

Notion is not considered a HIPAA-compliant system, so any personal identifying information or personal medical information that you enter into Notion is NOT stored in accordance with or protected by HIPAA regulations. Your doctor likely maintains a HIPAA-compliant storage and messaging system if that is important to you. The Simplest Medication Log is merely a template to show you one way you could structure the data and offers an easy way to enter it if you're not concerned with HIPAA compliance.


How do you use the Simplest Medication Log?

Installation

After going through the Gumroad purchase process you will be taken to a page similar to the one shown below. In the top right of that page click the "Duplicate" button (sign in if required).


The template will be duplicated into your personal Notion and listed at the top of the navigation menu on the left. It's installed! Click into the Simplest Medication Log and you will see the following page.

Add a New Entry

Click the left side of the "New" button in the top right (if you accidentally hit the dropdown arrow on the right instead of the left side of the button simply press the "Esc" key or click outside of the dropdown that appears and click again on the left side of the "New" button). A flyout form will appear. Click into each field and fill out the information as it makes sense to you. Once you've entered the information simply click out of the flyout and Notion has automatically saved your data! I've left a sample record in the database that shows you how I fill in the form, feel free to edit (click the item to open a flyout to edit the details) or delete it (right-click the item in any view and choose "Delete").

Viewing the Log

The template includes 3 views. You can use them however you like, or edit or delete or add new views through the options in the 3-dot menu next to the "New" button, but these are the ones I've found most helpful.

  1. Timeline
    1. As configured exposes the medication name, dosage, and status fields (excludes the Notes field) so I can quickly see how the medication is making me feel over time and spot trends.


  2. Log
    1. The filtering and sorting allows me to pick the data down to a very specific time period that I then export to a PDF that I can provide my doctor. Instructions on that below. All fields are shown.


  3. Table
    1. This is a convenient way to edit any previous entries where I've made a typo. All fields are shown.


Filtering

Filtering can be useful if different doctors have prescribed you different medications that you track separately so that you can give the doctor a log of only the medications that they would be interested in. You can filter by date in quite a few ways, but the most useful of which to me is to grab all log entries after my last appointment so they only see the newest data.

To filter select the "Filter" button on the right side above the entries. This should open a dropdown that allows you to select the field to filter by. We'll choose "Date" in this example. A calendar widget is opened. The first thing to do is check the top left of the calendar widget for a dropdown that says "Date is" and select the arrow next to it. Choose "Is on or after" instead. Then select the first day of logs you want to see. The filter is automatically applied, so click outside of the widget to close it and review your filtered log.


Sorting

By default the sorting should be setup in date descending order, newest entries at the top. You can adjust the sort by clicking the pill button on the left above the entries and below the views that tells you the field and direction being sorted. You can add additional sorts or sort by other fields by clicking the "Sort" button at the top, then the "Add Sort" button and selecting the options that interest you. I haven't used or needed any sorts other than the date descending, which is why I left it as the default.

Export to PDF

Once you have your filtered log press the 3-dot menu in the very top right of the page and find the entry labeled "Export" and click it.


This will open a popup with options. The default options as shown below are correct for me, but adjust as you need and click the "Export" button on the bottom right. This automatically downloads a PDF of the visible log into your Downloads folder that you can then print or send to your doctor.
